AIRCRAFT ACCIDENT INVESTIGATIONS |

11/6/05 - Air Traffic Expert, Party to NTSB investigation at Miami Center, FL. N5HU's in-flight break-up over Post Lucie, FL. Weather played a major role in this accident. The investigation reviled inconsistencies between requirements for approach controllers and center controllers in the issuance of weather information to airborne pilots. The investigation resulted in changes to the Airman's Information Manual and the FAA 7110.65 that corrected the differences.
03/28/05 - Air Traffic Expert, Party to NTSB investigation at Tallahassee, FL. Mid-Air Collision occurred while in communication with Tallahassee Approach control. A main point of the investigation involved Air Traffic Controllers' responsibility to issue traffic information to pilots.
06/07/04 - NTSB Hearing concerning FedEx1478 Major Accident in Tallahassee, FL.
05/04/04 - Air Traffic Expert, Party to NTSB investigation at Memphis Tower, FL. The pilot of N511WD lost control of the aircraft in the vicinity of thunderstorm activity. The Board found a contributing factor was insufficient information provided by ATC personnel.
01/08/03 - Air Traffic Expert, Party to NTSB Major Investigation involving the crash of a US Airways Express Beechcraft 1900 at Charlotte International Airport. Held the Air Traffic position on the NTSB ATC group for NATCA.
10/23/02 - Air Traffic Expert, Party to the NTSB Investigation involving the crash of a Cessna 208 in Mobil, AL. The investigation revealed spatial disorientation as a probable cause. Contributing to the accident was the night instrument meteorological conditions with variable cloud layers.
07/05/02 - Air Traffic Expert, Party to NTSB Major Investigation involving B727 that crashed at Tallahassee, FL. Held the position of NATCA Party Coordinator. A major aspect of this investigation centered on Controlled Flight in Terrain (CFT) also referred to as the Black Hole Effect.
02/06/02 - Air Traffic Expert, Party to NTSB Investigation involving the crash of a PA32 in Atlanta, GA. The investigation revealed spatial disorientation as a probable cause.
12/12/01 - Air Traffic Expert, Party to NTSB Investigation involving the crash of a PA32 at Jacksonville, FL. The probable cause was spatial disorientation causing a lose of control of the airplane during a missed approach
