Scope & Range of Services

Many "ATC experts" have not actively worked as Air Traffic Controllers in years.   This puts them out of touch with the current workings within the ATC field which significantly has effected aviation; the workforce has significantly changed - new changes in training, equipment and technologies along with new rules, procedures and regulations.  Patrick has worked through these changes (retired in late 2007), and as a result, adds perspective and experience to help accurately assess aviation accidents or questions. 

We will determine if FAA procedures and requirements were followed. Was training adequately given? Was the problem apparent and not addressed?  Were FAA procedures in error? What other factors contributed to the situation? These are some of the questions we can address in a clear manner that will assist attorneys in representation of their clients.

Years of experience listening to voice recordings and transcripts will provide clarity and understanding of the actual situation; often air traffic terminology is confusing and may be misinterpreted.

OVER 35 YEARS OF EXPERIENCE IN AVIATION:

 

  • A party member and coordinator with the National Transportation Safety Board (NTSB) representing the National Air Traffic Controllers Association (NATCA)

  • Participated in aircraft accident investigations as Air Traffic Control (ATC) Subject Matter Expert as a Group Member and Party Coordinator with the NTSB

  • Performed instructional activities concerning aircraft accident investigations to Air Traffic Controllers

  • Participated in Aviation Panels with the AOPA Vice President at Communicating for Safety Seminar with national members of the aviation industry in attendance

  • 15 years as a Full Performance Level (FPL) air traffic controller at Tampa Tower and Approach Control

  • 2 years as Air Traffic Manager of Albert Whitted Tower

  • 1 year as a Supervisor at Miami Tower and Approach Control

  • 1 year as a Staff Training Specialist at Miami Tower and Approach Control

  • 4 years as a Full Performance Level (FPL) air traffic controller at Miami Tower and Approach Control

1993-2007 - FAA      
Air Traffic Control Specialist, Tampa FL

 

  • Performed full performance level Air Traffic Controller responsibilities, as well as being an OJT instructor at Tampa Air Traffic Control Tower

  •  Served on the Facility Advisory Committee which consisted of upper management personnel and controller leaders; this committee steered the facility utilizing joint decision making

  • Served on Operational Error Reduction Team; wrote the Facility Administration Handbook

  • Wrote the Facility Standard Operating Manual; served as a leader on the Super bowl Committee which established procedures for handling the unprecedented amount of air traffic

  • Member of Tampa Tower Facility Evaluation Team for three years. Served as NATCA's Southern Region Air Safety Investigator (ASI) from 1998 until 2004 representing controllers, with Party Status to the NTSB, on accident investigations both locally and nationally where air traffic control was an issue

  • Served 10 years as NATCA representative working with management and representing the controller workforce when changing, implementing, and writing policies and procedures that applied to the controller workforce.

  • NATCA's Southern Region Air Safety Investigator (ASI) 1998-2004
    Southern Region NATCA Vice President's lead Air Safety Investigator
    Directed and coordinated NATCA Southern Region ATC Accident Investigation teams to accident sites. Provided On-the-Job Training to new NATCA Air Safety Investigators.

 

1991-1993 - FAA
Air Traffic Manager, Albert Whitted Tower, St. Petersburg FL

 

  • Responsible for the administration of a control tower; certified to work as an Air Traffic Controller in the tower and also worked as a first line Supervisor

  • Developed and implemented training manuals, Air Traffic Control Standard Operating Procedures, Facility Administration Handbooks and Quality Assurance programs

  • Responsible for all training including Air Traffic on-the-job training as well as career enhancement training

  • Responsible for ensuring the tower had all resources necessary including personnel, material and funds

  • Certified as a Tower Examiner for the purpose of issuing Control Tower Operators Licenses. Participated in airport meetings with local officials

  •  Was a focal point for the FAA Combined Federal Campaign to accept donations for its charities

  •  Participated in Equal Employment Opportunity forums.

 

1990-1991 - FAA
Air Traffic Supervisor, Miami, FL

 

  • Served as a first line Supervisor at Miami Tower and Approach Control

  •  Responsible for Air Traffic Operations during my watch which included traffic safety and traffic management

  •  Other responsibilities included ensuring Air Traffic Controllers in training received proper training including on-the-job training and career enhancement training, evaluating and rating trainees and full performance level (FPL) controllers and issuing awards and other recognition

  •  Developed a local facility course titled "How to be a Supervisor" for Miami Tower. This included its development and production and was delivered in book format with slides for presentation. As a result the highest On-the-Spot award the FAA allowed was issued.

Air Traffic Control Staff Training Specialist, Miami FL

 

  • Served as Staff Training Specialist at Miami Tower and Approach Control

  •  Responsible for refresher, supplemental and remedial training of all facility trainees and full performance level air traffic controllers.

Air Traffic Control Specialist, Miami  FL

 

  • Served as a Full Performance Level Air Traffic Controller at Miami Tower and Approach Control

  •  Was responsible for the separation of aircraft in a Level 5 Control Tower and Level 5 Approach Control

AIRCRAFT ACCIDENT INVESTIGATIONS

 

11/6/05 - Air Traffic Expert, Party to NTSB investigation at Miami Center, FL. N5HU's in-flight break-up over Post Lucie, FL.
Weather played a major role in this accident. The investigation reviled inconsistencies between requirements for approach controllers and center controllers in the issuance of weather information to airborne pilots. The investigation resulted in changes to the Airman's Information Manual and the FAA 7110.65 that corrected the differences.

03/28/05 - Air Traffic Expert, Party to NTSB investigation at Tallahassee, FL.
Mid-Air Collision occurred while in communication with Tallahassee Approach control. A main point of the investigation involved Air Traffic Controllers' responsibility to issue traffic information to pilots.

06/07/04 - NTSB Hearing concerning FedEx1478 Major Accident in Tallahassee, FL.

05/04/04 - Air Traffic Expert, Party to NTSB investigation at Memphis Tower, FL.
The pilot of N511WD lost control of the aircraft in the vicinity of thunderstorm activity. The Board found a contributing factor was insufficient information provided by ATC personnel.

01/08/03 - Air Traffic Expert, Party to NTSB Major Investigation involving the crash of a US Airways Express Beechcraft 1900 at Charlotte International Airport.
Held the Air Traffic position on the NTSB ATC group for NATCA.

10/23/02 - Air Traffic Expert, Party to the NTSB Investigation involving the crash of a Cessna 208 in Mobil, AL.
The investigation revealed spatial disorientation as a probable cause. Contributing to the accident was the night instrument meteorological conditions with variable cloud layers.

07/05/02 - Air Traffic Expert, Party to NTSB Major Investigation involving B727 that crashed at Tallahassee, FL.
Held the position of NATCA Party Coordinator. A major aspect of this investigation centered on Controlled Flight in Terrain (CFT) also referred to as the Black Hole Effect.

02/06/02 - Air Traffic Expert, Party to NTSB Investigation involving the crash of a PA32 in Atlanta, GA.
The investigation revealed spatial disorientation as a probable cause.

12/12/01 - Air Traffic Expert, Party to NTSB Investigation involving the crash of a PA32 at Jacksonville, FL.
The probable cause was spatial disorientation causing a lose of control of the airplane during a missed approach

03/30/01 - Air Traffic Expert, Party to NTSB Investigation involving the crash of a PA32 in Concord NC.
