In the past three years, Mr. Bristow has filed only twice for permission to appear in Florida, in the proceeding below and in Jacobs v. Seminole County Canvassing Board, Circ. Ct. Case No. 00-2816, DCA Case No. 1D00-4832, SC 00-2447. Mr. Bristow has never been disciplined for matters pertaining to his practice of law. 44. Ms. Maddux is and has been an active member in good standing of the Bar of the Court of Appeals of the Texas. In the past three years, Ms. Maddux has filed only once for permission to appear in Florida, in Jacobs v. Seminole County Canvassing Board, Circ. Ct. Case No. 00-2816, DCA Case No. 1D00-4832, SC 00-2447.. Ms. Maddux has never been disciplined for matters pertaining to her practice of law. 55. Mr. Levey is and has been an active member in good standing of the Bar of the District of Columbia and Illinois. In the past three years, Mr. Levey has filed only twice for permission to appear in Florida, in the proceeding below and in Jacobs v. Seminole County Canvassing Board, Circ. Ct.
